If you have an immediate need or if you are in crisis, please call 888.243.5744 to speak with a clinician right away.

If you are seeking a counseling referral to set-up face-to-face counseling with a licensed clinician in your area, please fill out the intake form and submit. An EAP Intake Consultant will respond to your request by calling you at your desired contact number within 1 business day. Office hours are Monday-Friday 7:30am-5:00pm CST.
